During my pharmacy practice residency, I have been trained to take the responsibilities of a decentralized clinical pharmacist. These responsibilities include responding to STEMI, stroke, trauma, and cardiac arrest codes. I have also been trained to cover clinical services in medical and critical floors by participating in rounds, verifying orders, responding to consults, answering questions, making interventions; including but not limited to; preventing medication induced allergic reactions, converting IV to PO, PKs monitoring, and recommending antibiotic therapies for adults and pediatric population. Moreover, I had ambulatory care rotations at which I worked in diverse settings such as; outpatient/indigent care clinic, long-term care, and oncology outpatient clinic, which enabled me to gain the autonomy to start new therapy and monitor and adjust therapeutic plans. This integration between the acute care setting as well as the outpatient care setting has provided me the opportunity to excel in the field of primary care and acute care.
